Puppy Adoption Contract

This agreement is made between Davis Family Doodles (“the Breeder”) and ________________ (“the Buyer”).

The balance for the puppy is due on or before the go-home date.

Davis Family Doodles charges a total deposit of $500. If placing a deposit after the puppies are born, you will need to pay your full deposit towards the puppy of your choice if puppy picking has already begun and deposits are not transferable to a different puppy once your puppy has been chosen. Puppy picking will be done at the time of the breeder’s choosing but not later than six weeks of age and will go in the order the deposits are placed. This allows for adequate time to make sure all puppies are thriving and that it is the best time to allow buyers to choose the right puppy. If you are placing a deposit prior to a litter being born or a pregnancy being confirmed, your first payment of $200 is paid at the time of being placed on the waiting list and insures your spot on our list. Once the puppies are born and you are notified of genders, colors, and your spot in the line, you will be able to choose if you would like to stay on this litter’s list and choose from the available puppies or move to another litter. If you choose to pick your puppy at that time, you will need to place an additional deposit of $300 towards your total $500 deposit. The remaining balance of the puppy will be paid at pick up and your deposit will be counted towards the full balance of the puppy. If you wish to move your spot to a future litter, you may do so at any time prior to picking a specific puppy.
All deposits are non-refundable unless Davis Family Doodles has not been able to provide you with the specific puppy that you had placed a deposit on, or if we are unable to provide you with a puppy of your desired breed, color and gender within one year. At one year, if you have not been offered a puppy of your desired breed, color and gender, you will be refunded your full deposit.

Missouri sales tax of 6.5% will be added to the final purchase price of the puppy. Payments made that are not done using friends and family option, will be charged an additional 3% for the service fee. All credit card fees associated with paying with a credit card will be charged to the buyer.

HEALTH
The puppy goes home with age-appropriate vaccinations, deworming, and a health record. The Breeder warrants the puppy is in good health at go-home; the Buyer agrees to a licensed-vet exam within _3___ days.

Davis Family Doodles would never knowingly sell an unhealthy puppy and all puppies receive a health check from a licensed Veterinarian prior to being placed in their new home. We guarantee our puppies to be free from life threatening illness or disease for the first 3 days after picking your puppy up. After the 3 days, your puppy could have contracted an illness from its new environment or on the travels to its new home and therefore we cannot guarantee it further. You will need to have your puppy checked out by a licensed Veterinarian within 3 days of picking your puppy up to ensure the puppies health. If you do not have a check- up within 3 days, our 3 day guarantee will be void. If your puppy is found to have a life threatening illness or disease, we will refund you the purchase price of the puppy not including any shipping. Davis Family Doodles will not be responsible for any treatment you may decide to give your puppy.

CONGENITAL/HEREDITARY ADDITIONAL COVERAGE: 
We additionally guarantee the puppy for two years to be free from any severely life altering genetic illness. If you choose to keep your puppy, we will refund you one half of the purchase price of your puppy not including shipping cost. If you choose to return your puppy to us, we will refund you the full amount or you can chose to replace your puppy with another from Davis Family Doodles of equal value. Any refunds will not include any shipping cost and the buyer will be responsible for returning the puppy. All guarantees will only be valid if proof of the diagnosis is provided by a Licensed Veterinarian. Additional documentation may be needed such as proof of testing, x-ray, etc. The documentation will need to have an address and phone number for Davis Family Doodles to contact the veterinarian and the veterinarian will need to be acting in standard protocol for the situation. Davis Family Doodles reserves the right to consult our Veterinarian over the issue and take their opinion and the opinion of the puppy’s veterinarian into consideration. This guarantee does not cover Coccidia, Giardia, or Worms because they are so common in puppies. We do deworm puppies every two weeks to do our best to send you a worm free baby. We also do a stool sample from the litter to check for any additional steps we may need to take prior to your new puppy coming home. We do recommend getting a stool sample tested at your initial veterinary visit to ensure your puppy is free of any parasite. It also does not cover coloration changes, adult size, or behavior of the puppy once it leaves our care.

SPAY / NEUTER 
This puppy is sold as a companion only and will not be bred unless agreed otherwise in writing. The Buyer agrees to spay/neuter by _10___ months. The buyer agrees to not breed the  puppy and will provide Davis Family Doodles with documentation from the vet showing the spay/neuter of the pet within the first year of the puppy’s life. If the buyer choose to breed the puppy without the consent of Davis Family Doodles, the buyer will surrender all puppies and the mother to Davis Family Doodles. If I do not abide by the above, the buyer will pay a penalty of $15,000 plus court fees incurred by Davis Family Doodles to uphold this contract.

5. LIFETIME RETURN
If the Buyer cannot keep the puppy at any point in its life, the Breeder must be contacted first and has first right of refusal. This puppy may never be surrendered to a shelter or rescue.

6. ADDITIONAL TERMS
__ This adoption agreement is for the life of the puppy. The buyer agrees to provide the puppy with adequate food, water, shelter and medical care including, but not limited to, flea and tick, deworming, and heart worm medications. The seller has the right to visit the buyer’s home no more than four times within the first 12 months of my puppy’s life to confirm that I am providing proper care. If, for any reason,  the buyer is unable to provide this care for the puppy, the buyer will return the puppy to the seller on a non-refundable basis or the buyer will re-home the puppy to a home approved by Davis Family Doodles prior to relocation. If Davis Family Doodles does not approve of the new home the puppy is to be placed in, the puppy will be returned to Davis Family Doodles at the buyer’s expense. The puppy will under no circumstance be surrendered to a shelter. The buyer understands that Davis Family Doodles is not responsible for any behavior of the puppy after the puppy has left Davis Family Doodles and that a refund will not be given for any behavioral reason.

I have read the Davis Family Doodles Adoption Contract, Health Guarantee and Deposit
Contract and my signature states that I understand and agree to the terms and conditions. I also understand that failure to abide by the Adoption Contract will void any and all Health and Genetic Guarantees and that I, the buyer, will be responsible for returning the puppy at my own expense to Davis Family Doodles.
